A3 TEACHING ARTISTS ONLY

You are covered under SAIF Workman’s Compensation Insurance. If you are injured it is your responsibility to contact Mike Fisher and Carolyn Hatefi ASAP. SAIF reports are due in a timely manner to ensure that you are taken care of quickly. If you are injured and go to the doctor, let them know that you work for the Academy of Arts & Academics and that SAIF is your workman’s comp company. They will fill out form 827. You will need to fill out the 801 form if you chose to start of claim and an Incident form whether you chose to start a claim or not. You will need to turn all the forms, including copies of the doctor’s forms in to Carolyn ASAP.

The doctor’s report will let A3 know if and when you can return to work and if you are on any restrictions. You must have the forms to Carolyn before you can return to work.

 

INJURED NOT ON THE JOB

If you are injured not while at work that required you to see a doctor, please bring a doctor’s note to Carolyn with permission to return to work. If there are any restrictions, the note will need to outline them.
